The valley of tears almost sounds poetic but in aesthetic medicine it is a reality of unwelcome truth. It is located beneath the eye, and we often confuse it with dark under-eye circles, in fact it is the hollow that gradually forms over time, extending downward from the dark under- eye circles.

In fact, the dark circle is the horizontal half-moon (jugal-palpebral groove located between the lower eyelid and the cheek), often brownish in tone, while the valley of tears is more of a vertical dip or hollow.

The tear trough is the hollow, which is situated between the cheekbone and the nose, below the under-eye circle and descending toward the mid cheek. It gives the face a tired and sad appearance which only disappears while smiling, since the smile lifts the corners of the mouth.

What are the causes?

Bone, muscular and fat changes come with age and trigger the appearance of this tear trough.

At this facial zone level, fat sags and disappears over time, a hollow forms which gives a tired and sad facial appearance.

Aging, genetics, tobacco, and sleep deprivation can also trigger the phenomenon. These grooves hollow out more and more giving the face a tired appearance. To restore radiance and volume to the face, Doctor Benjamin Cozanet uses dermal filler products at his aesthetic medical clinic in Paris.

In the event of weight loss, the tear trough hollows out even more, and with volume loss to the cheeks, it all gives the face a skeletal appearance. Treating this zone is a major factor contributing to facial rejuvenation or simply to restore a healthy look and equally to bring back the desired “V” shaped face, a symbol of youth and elegance.

How does Dr Benjamin Cozanet eliminate the tear trough ?

For Dr Benjamin Cozanet, it is essential to treat the tear trough before the nasolabial folds because injecting hyaluronic acid into the tear trough will restore facial harmony, revitalize the gaze (below under-eye circles) and will produce a “mini lift” effect which corrects cheek sagging and thus improve the nasolabial folds.

To replenish lost volume to the tear trough, extra caution is required to not result in an excessively plump cheek effect while smiling.

For this very reason, Dr Benjamin Cozanet uses hyaluronic acid injections from the world’s best aesthetic medical laboratories (Allergan Juvederm, Galderma Restylane, Merz Aesthetics Belotero et Radiesse).

The hyaluronic acid injections can also complement an under eye shadow filler treatment by extending to the valley of tears. However, a different type of hyaluronic acid is used because the valley of tears requires a more volumizing type of acid (without being too volumizing, in comparison to the one used to restore disappearing cheekbones), while under eye circles require a less hydrophilic hyaluronic acid to avoid any edema issues or under-eye puffiness.

The radiance and glow restored to our lower eyelid zone will bring the irresistible charm back to our gaze, a major “weapon of seduction”. In the case of moderate sagging at cheekbone and cheek level, Dr Benjamin Cozanet could equally prescribe the insertion of non-surgical thread lifts to reinforce this zone.

If the tear trough is accompanied by a wrinkled skin phenomenon at cheek level, the treatment may be combined with a Skinboosters injection or a soft-tissue filler (Radiesse®, Calcium hydroxyapatite / or Sculptra® or HarmonyCa®, and/or an Ultherapy Prime® session to improve skin density and the natural amount of collagen and elastin in the skin and/or EMFACE®

The tear trough filling process

The latest injection techniques provide simple, effective and safe treatments of the tear trough.

Dr Benjamin Cozanet employs flexible, blunt tipped microcannulas which leave no trace of injection, and in which no social downtime is required.

This treatment yields immediate results, the face appears naturally relaxed and refreshed.

Combo treatments

Dr Benjamin Cozanet may treat an overly hollow under-eye circle at the same time, or later, depending on the patient’s profile. The cannula is also used to avoid bruising and swelling by using a small dose of hyaluronic acid which is lightly or non-reticulated, and less hydrophilic to avoid swelling or puffing and to be able to resume normal activities with minimal social downtime.

In certain cases, it is advised to schedule a cheekbone treatment (zone located just above), to fully treat the zone in aesthetic medicine referred to as the “middle third of the face”, to provide complete, natural and harmonious results along with the improvement of the facial oval, if this zone has already begun to show signs of sagging.

The wrinkled skin across the cheeks can be treated with a painless injection by “multilayering” or with hyaluronic acid -based mesotherapy (Skinboosters) which plump the skin by deeply rehydrating without modifying facial volume; or in certain cases with Calcium hydroxyapatite (Radiesse®), or with Sculptra® or with HarmonyCa®, to increase the natural production of collagen and provide a smoothing and lifting effect to the cheek zone.
